If your child requires medication administered to them in the school day, you must complete the form below. Please give all medication directly to the class teacher. Medication cannot be administered without the completion of this form
Children are not permitted to carry their medication with them.

Parents are responsible for ensuring the information we hold about your child's asthma is up to date. You must also ensure that the inhaler held in school is in date.
